Registrations for the upcoming year are every Tuesday from 9:30 a.m ~ 11:30 a.m. In order for your child to be included for the Kindergarten Orientation in June, you must register in a timely manner. Child must be five years old by January 1, 2009 to enter Kindergarten in August 2008. Call Prendergast School at 736-5080 if you need any additional information.

Kindergarten Registration Information-for 2008-2009 school year

You must have the following information/documentation at time of registration:

Birth certificate with the notarized seal. It may be a copy as long as it has the seal.
A minimum of two proofs of residency, which include:
Valid Connecticut drivers license indicating Ansonia residency on the front of the license.
Deed to home or dated rental agreement showing student(s) name.
Escrow papers or signed mortgage commitment.
Utility bills (dated within the last three months).
Notarized letter from landlord or owner acknowledging parent/guardians and student’s residency.
Address verification from the Ansonia Post Office (NOTE: There may be a post office fee, payable by the applicant, for this service).

3. The blue health form completed by parent/guardian and physician at the time of the physical. If you would like to register before your child goes for a physical examination, you will need to bring a copy of all of the child’s immunizations. No child will be allowed to enter school without having the blue health form completed.
