Return to District Model Form

Students participating in the voluntary remote learning option may return to in-person attendance by completing the Return to District Model form. Parents/Guardians should remember when the completed the original form they agreed to keep their child out for the first marking period. The District has created an appeal process to allow for a change if your situation has changed. We are asking families to complete one form per child. Requests to return to the district model may take up to ten (10) school days to process in order to ensure that the District maintains the necessary class cohort sizes. Although the District will strive to keep the same placement/schedule as the one your child has in remote, it may not be feasible to do so and your child's return to school may involve a change in teacher or class schedule.
